Patient Guide to Our Services

 

Welcome.  We are glad you chose The Family Health Centers of Georgia, Inc. as your provider of choice and medical home.  The  Patient Guide to our Services is designed to assist you with understanding what will happen before, during and after your visit.  Your healthcare is our priority.  We pledge to provide you with quality healthcare that is delivered in a clean and safe environment.

Our Patient Portal

eClinicalWorks is a patient portal service that eliminates time-consuming phone calls and allows you, the patient, on-line access to your physician’s office.

With respect to system security, unlike emails that use multiple servers over the internet, the eClinicalWorks technology allows your doctor to use a single server secured by both a firewall and 128-bit encryption to safeguard your privacy. This secured system operates by a password protected log-in that you receive upon registering for the service.

Only you will have access to your information. Each family member will receive their own log-in code and password. If the patient is under 18 years of age, the responsible parent or legal gaurdian will receive the enrollment code and password information.

After Hours Care

If you are experiencing a medical emergency, please call 911.  

Our goal is to ensure our patients have continued access to the medical care you need.  If you are an existing patient and have a medical concern after hours that you needs immedidate attention from your medical provider, please call 404.752.1400 and follow the telephone prompts to reach your provider by department.

Our normal business hours are Monday - Friday from 8:00 a.m. - 5:00 p.m.  We are open late at the Main Center on Mondays and Thursday until 9:00 p.m.  Hours of operation vary based upon location.  Go to the "Find a Location" tab to see the hours of operation for the center you visit.

  • The earlier you call us, the better the chances are that you will get your desired appointment date and time.  Please ask the appointment representative to check availability at other locations and with other providers if your preferred location and/or provider is not available on your desired date.

We stive to get you the care you need as soon as possible.  You may also walk-in at any of our center locations during normal business hours.  Our professional team will make sure your healthcare needs are addressed that day or schedule you for the first available appointment.

New Patient Registration

New patient registration is processed at the center you visit.  As a new patient, we ask that you arrive at least 30 minutes prior to your appointment time. Please bring your current photo identification, proof of residency and payment for services. Paying for Services

The Family Health Centers of Georgia, Inc. gladly accepts the following methods of payment for services:

  • Medicare/Medicaid
  • Most private insurance plans, including Humana, Blue Cross Blue Shied, Aetna,  HMO, PPO, POS
  • Most major credit cards & debit cards
  • Cash/Check (Check payments are processed instantly via ACH)

Please bring anticipated funds for co-pays and deductibles at the time of service.

Need AFFORDABLE health insurance or to EXPAND your existing health insurance?

Applying for the ACA Health Insurance Marketplace.  You have questions, we have answers!  It’s the law.  All eligible individuals are required to obtain health insurance. Insurance plans will cover doctor visits, hospitalization, emergency room care, prescriptions, maternity care, and more .FREE information & enrollment assistance is available to help you choose the plan that works best for you!

Sliding Fee Scale Program

FHCGA receives a portion of its funding from the US Department of Health & Human Services. This funding allows us to offer a Sliding Fee Scale Program to uninsured and under-insured patients. The chart below (base upon 2013 guidelines) is for informational purposes only and does not guarantee acceptance into the program and/or charges for services. All applications for the Sliding Fee Scale Program must be completed in person. Applications are accepted at the time of service.

Translation, Interpretation & Handicap Accessiblity

We care about every patient and the supporting services that you may need to make your visit a success.  The Family Health Centers of Georgia has access to professional translation and interpretation services to help you navigate our services.  

Translation services are available for Spanish, French and multiple other languagues.  Sign language and options for the hearing impaired are also available.  

Please let us know at the time you make your appointment and/or at the time of your visit that you need on-site assistance.

UNDERSTANDING YOUR HEALTH RECORD/INFORMATION

Each time you visit a hospital, physician, or other healthcare provider, a record of your visit is made. Typically, this record contains your medical history, symptoms, examination and test results, diagnoses, treatment, care plan, insurance, billing, and employment information. This health information, often referred to as your health record, serves as a basis for planning your care and treatment and is a vital means of communication among the many health professionals who contribute to your health care. Your health information is also used by insurance companies and other third-party payers to verify the appropriateness of billed services.

Patient Rights

As a patient you will be treated in a manner that does not unlawfully discriminate against you based on your race, color, national origin, religion, sex (including pregnancy), age, sexual orientation (including gender identity and expression), marital status, disability, veteran status, or any other basis prohibited by federal, state, or local law.

 Medical and Dental Care

  • As a patient you will be treated in a manner that does not unlawfully discriminate against you based on your race, color, national origin, religion, sex (including pregnancy), age, sexual orientation (including gender identity and expression), marital status, disability, veteran status, or any other basis prohibited by federal, state, or local law.
  • As a patient you have the right to receive considerate and respectful care.

  • You have the right to refuse treatment to the extent permitted by law and government relations; however, you will be responsible for the consequences of refusal.

Respectful Treatment

  • As a patient you have the right to considerate and respectful care, with recognition of personal dignity, which takes into consideration your personal values and beliefs.

Privacy and Confidentiality

  • As a patient, within the federal HIPPA laws and state requirements, you are entitled to privacy and confidentiality concerning your medical care.
  • As a patient, you have the right to expect that all communications and records pertaining to your care will be treated as confidential. You have the right to approve or disapprove the release of any disclosures of records, except when the release is a legal requirement. You also have the right to access information contained in your medical record.

 Identity

  • As a patient, you have the right to know, at all times, the identity, professional credentials of the healthcare personnel, as well as the name of the healthcare provider primarily responsible for your care.

Explanation of Care

  • As a patient, you have the right to an explanation concerning your diagnosis, treatment, procedures, and prognosis of illness in a language that you can understand, so that you will be able to make decisions about your care, treatment, or services.
  • As a patient, you have the right to request assistance in formulating Advance Directives.

Informed Consent

  • As a patient, you have the right to be advised in non-clinical terms of information needed to make knowledgeable decisions on consent or refusal of treatments. Such information will conclude significant complications, risks, benefits, and alternative treatments available.

Research Projects and Surveys

  • As a patient, you have the right to be advised if The Family Health Centers of Georgia, Inc. proposes to engage in or perform in research projects or surveys associated with your care or treatment. You also have the right to refuse to participate in any of these projects or surveys.

Safe Environment

  • As a patient, you have the right to care and treatment in a safe environment.

The Family Health Centers of Georgia, Inc.’s Rules and Regulations

  • As a patient at The Family Health Centers of Georgia, Inc. you have the right to be informed of The Family Health Centers of Georgia, Inc.’s rules and regulations that relate to patient or visitor conduct. You are entitled to this information during the interview process and during resolution of complaints.

 PATIENT RESPONSIBILITIES 

Providing Information

  • As a patient you have the responsibility to provide, to the best of your knowledge, accurate and complete information about complaints, past illnesses, hospitalizations, medications, and other matters relating to your personal health.

Respect and Consideration

  • As a patient, you have the responsibility to be considerate of the rights of other patients and The Family Health Centers of Georgia, Inc.’s healthcare personnel. You are responsible for being respectful of The Family Health Centers of Georgia, Inc.’s property of other persons and of the facility.

Compliance with Medical Care

  • As a patient, it is your responsibility to comply with the medical and nursing treatment plans, including the follow-up care recommended by your healthcare provider.

Medical Records

  • As a patient, it is your responsibility to ensure that medical information is promptly returned to The Family Health Centers of Georgia, Inc.’s Medical Records Department for appropriate filing and maintenance when you transport those records for the purpose of medical appointments or consultations, etc.

The Family Health Centers’ Rules and Regulations

  • As a patient, you are responsible for the following The Family Health Centers of Georgia, Inc.’s rules and regulations affecting patient care.

Reporting Complaints

  • As a patient, you are responsible for helping The Family Health Centers of Georgia, Inc. provide the best possible care to all of our patients and visitors. Your recommendations, questions, or complaints should be reported to the immediate supervisor of the service area. If you need additional assistance, you have the right to request to speak with the Risk Manager.
